Please call your insurance company to verify we are in network with your plan, or if you are allowed to see us out of network. If your plan requires a referral, please obtain one prior to making an appointment with us.
All varicose vein procedures must be pre-certified or pre-authorized for medical necessity with your insurance company before we begin to set up your procedure date. Our surgeon will discuss your eligibility and specific benefits for the recommended procedure(s) during the treatment plan discussion at the initial consultation. We will submit the required documents to the Medical Director of your insurance company: Ultrasound test results and photos or drawings of your veins. These documents will establish the medical necessity of the procedures.
Most insurance companies and Medicare cover the office-based Endovenous laser treatment (EVLT), mini phlebectomies, and compression sclerotherapy to treat varicose veins of patients experiencing symptoms of venous insufficiency and venous hypertension.
It is important for you to know that while your insurance plan may approve your procedure(s), the entire cost of it may not be fully covered due to co-insurances, co-payments, or deductibles that have not been met. The balance over or above the allowable cost will be the patient’s responsibility. Therefore, it will be in your best interest to verify insurance coverage and/or non-covered costs before scheduling the procedure.
Sclerotherapy for spider veins without symptoms is considered by most insurance companies to be cosmetic or not medically necessary treatments. Therefore, if your insurance company does not approve the sclerotherapy procecdure(s), we will offer you a reasonable out-of-pocket cost or installment payment plan if you still wish to have it done.
